Prizes & Awards

Prizes & Awards - for monetary & nonmonetary prizes & awards, including gift cards and other funds awarded to students and community members.   Gift cards should not be issued to state employees. 

• Minnesota State Substitute W-9 - for monetary & nonmonetary prizes & awards and other funds provided to students & community members.  May be used to collect information for monetary payments as well.
• Prize & Award Reporting spreadsheet
• Prize & Award Tax Residency Information Form (TRIF)   -  to be completed by individuals who are not U.S. citizens or permanent resident aliens to determine if the prize/award is subject to federal nonresident alien tax withholding and reporting.  Send the completed TRIF and supporting immigration documents to Tax Services.  For an individual present under F-1 nonimmigrant student status, the following immigration documents must be gathered (for the documents needed from individuals present under a different immigration status, contact Tax Services):

  • Unexpired foreign passport identification page, U.S. visa page and any other pages with U.S. immigration stamps.

  • I-94 arrival/departure record & I-94 travel history

  • I-20 SEVIS form
